Important Information for Entry in His Oaks
Please note that entering His Oaks involves more than just submitting a Participant Application; the following forms also need to be completed. These can be printed, signed, and either mailed or emailed to His Oaks Ministry.
If this is not feasible, don’t worry! His Oaks will have these forms available for you to fill out at the facility. We encourage you to review them before your first day so that you understand their contents and can ask any questions you may have.
